Physical Examination
Students registering for Athletics must have a current physical examination on file in the Activities office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and uploaded prior to the first day of practice/tryouts. Completed physicals MUST be uploaded in “Step 4: Physical Forms” of the online registration form.
Athletes will NOT be allowed to try out or practice until the following items are completed:
- Online Registration
- Current Physical from Doctor uploaded - form located on Atheletic Home Page, valid 365 days from date of last exam, must be a clearance for SPORTS participation.
- Athletic Fee paid at Business Office, located in the Main Building, 2nd floor. Hours: Monday - Friday from 7am - 3:30pm
- Clearance by the Athletics Office - yellow clearance slip given to athlete at the Athletic Office after steps 1-3 are complete.
